在Pandemic中，多種惡毒的疾病同時間在世界各地爆發！玩家在遊戲中擔當抗疫專家，目標是應付疾病爆發熱點，研究四種瘟疫的治療方法，防止它們失控。

主遊戲板標示了地球上多個主要城市，玩家每一輪可以作出最多4個行動，包括在城市之間遊走、治療受感染的群眾、發現一種治療方法及興建一個研究站。玩家可以從抽取到的卡牌中獲得這些能力，但在整副卡牌中卻散佈着流行病卡，抽到時會引致疾病加速及加強，而第二副卡牌則負責控制病毒正常傳播。

玩家在團隊中扮演獨特角色，他們必須計劃好策略，結合角色的專業能力優勢以戰勝病毒；例如營運專家可以興建研究站，用以尋找治療疾病的方法，同時容許城市之間的流動；科學家只需要4張特定疾病的治療卡便可治癒該病，不是平常的5張。但疾病傳播迅速，時間不多，如果一種或多種疾病蔓延到無法處理，又或者玩的時間過長，玩家們便輸了；若果治癒了4種病，玩家便勝出。
